I've seen it drop twice in korafax (non-instance) near the telivar area / outside of the mindshear avatar prep point. I'm sure it can drop in any tier 5 group zone but i've seen it drop here twice in 18 hours of playtime a few days ago.

I saw it drop from a trash mob in OBF once and I know I saw it somewhere else but I can't remember where now.
