I suggest taking a look at the pet hold button on your pet window.
Click your pet hold hot button and see if the pet window button is actually lighting up or just pushed down for a second then going back out.

When I raid I want my pet hold on all the time to prevent accidental pet agro.

When I solo i want pet hold off so the pet will engage any mobs that hit me, because when solo I don't want to forget that pet hold is on and wonder why my pet is not taking agro.

So my /pet hold hot button allows me to call my pet back if I need to reel him in when i solo and he will automatically start fighting when the mob comes to camp and hits him.
The hot button also allows me to stop my pet from fighting and clear agro list when raiding because it leaves the pet hold button in my pet window with the same configuration it had before i hit the pet hold hot button.

/pet hold
/pet back off
/pet hold

Just have to make sure that the pet hold button is set the way you want before starting raid mode or solo mode.

Why?
Because the old pet hold was not broken and regroup and the rest were not needed to fix it.

My opinion,

Mine is:

/pet regroup on
/pet back off

Works flawlessly every time.

Ditto...
Played with a few combos and this works good for me as well.

I thought one of the objectives of the new pet window was so that while in raids a mage does not have to hit a "pet hold" key after every mob.

I only want my pet to attack a mob that I /pet attack, until it is dead, or I /pet back. If /pet attack a different mob while it is still on mob 1, I do not want it to go back to mob 1 unless I /pet attack again.

Is this possilbe using the standard pet keys?

If you toggle Greater Hold on it will only attack things you tell it to attack. To switch targets and clear the first target from your pets hate list, you could create a hotkey that says:

/pet back off
/pet attack

Which could also double as your back off key if you clear your target and then hit it.

That is helpful as it is better than hitting a key after every mob. The only thing that stinks about it is you can not do it within the Pet Info window, and all 4 of my hotkeys are filled up so I will have to drop one. Annoying, but better than nothing.

So on raids, I have /focus /follow /spellhold /ghold toggled on for water pet, and have a hotkey set up with /pet back off /pet attack for attacking mobs.

Okay... people are misunderstanding, I think, that there are now TWO TYPES of pet hold. There is pet hold (normal) and ghold (greater hold).

Pet hold (normal) means it won't attack until told. Once it attacks, anything aggroing the pet gets on the list and pet keeps going until you hit regroup (back only calls it back until it gets reaggro'd).

Greater hold means it won't attack until told AND it will come back after it has killed that mob. There is a caveat: if you tell it to attack another mob before the 1st has died, it will attack mob 2 and continue until mob 2 dies, then GO BACK to mob 1. This list is infinite and it will go to the next mob each time you tell it to attack. This is what you should be using if you want your pet to come back after the mob is dead without hitting another key. If you accidentally build up a hatelist, just hit pet regroup until you have a mob you want to kill then hit pet attack.

So what use is pet back? I use it if I'm switching targets permanently. A good example is Tjudawos in cryst. Initially, he's a burn until 80%(?) then adds spawn the rest of the event. So, I have ghold on, pet goes in to named during burn period but after that I have to switch to killing adds. If I don't use pet back, he will go back to Tjudawos after each add dies. I have a reason not to want this so once the adds spawn, I hit pet back and go about my business.

Pet Regroup is what you use if you want your pet to come back whether something is beating on it or not. It's a toggle so it is NOT clearing the hatelist. What it does is make your pet come to you no matter what. Hit back to clear the hatelist, hit attack to turn off pet regroup and make it attack a new (or the same) target, or hit regroup again to turn off the toggle and pet will go back to killing the target(s) on its hatelist.

Not entirely accurate. Pet hold is a toggleable stance now, not a one off command. So if pet hold is toggled on, hitting back off will hold your pet. The only point of difference between regroup and back off + hold is the retention of the hate list.
